EHCF Annual Grant Final Report

This final evaluation form needs to be submitted at the end of the project that you were awarded grant monies from the Enhance Hamilton County Foundation. Reports must be returned upon completion of the grant project on or before December 31 or you are not eligible to apply for another grant.
