Final Annual Grant Report

If you were awarded a Grant and have completed your project, please submit a final report form.

In order to be eligible for a new grant, you must have all your previous grant projects completed and final reports turned in.

If the project cannot be completed by the end of the year of grant funds were received, grant recipients must request an extension
on the project via email stating the reasons the project is not complete and when completion is anticipated.

In that case, a written report is required upon completion of the project.

If no grant extension is requested, the full grant amount must be returned to the Enhance Hamilton County Foundation.
